Cover illustrations: The photograph is copyrighted 1989 by The State Journal Register. It is of White Hall Hospital, which closed last summer. It's closing and the closing of other Illinois hospitals are symptoms of greater problems in the delivery of health care in rural Illinois. The photograph was reprinted with permission of the publisher. The photographer is Rich Saal.
