Organ donor cards stickers available for drivers licenses

UNDER the "Uniform Anatomical Gift Act" of 1975, the Secretary of State's Office was given the responsibility to distribute "Organ Donor" stickers to drivers who have made previous arrangements with a donor organization. The sticker is then affixed to the upper right hand corner of the back of the drivers license. The only purpose of the sticker is to alert authorities that the driver is also carrying an Anatomical Gift Card, These cards are provided by the following:

Kidney Foundation of Illinois, Inc.
127 North Dearborn Street
Chicago, Illinois 60602
Telephone: 312/263-2140

Lions of Illinois
Eye Donor Registry
1515 North Harlem Avenue
Oak Park, Illinois 60302
Telephone: 312/383-1900

Illinois Society for the Prevention of
Blindness, and Illinois Eye Bank
53 West Jackson Blvd. - Room 1435
Chicago, Illinois 60604
Telephone: 312/922-8710

Persons wishing to donate their eyes or kidneys should contact one of these associations or any other of their choice. The program's purpose is to get the organs to a bank before they become oxygen starved, The drivers license, which is always examined after a fatal accident, is a logical way to accomplish this.

Cards and stickers may be obtained from either a local Drivers License Facility or from one of the organizations. These cards must be signed by the donor and, in some cases, by witnesses. The actual donation's made 10 a bank of the individual's choice and does not involve the Office of the Secretary of State.
